The Forms Directory contains electronic versions of many forms commonly used at the University.

Forms exist primarily in Adobe PDF, Microsoft Word and Microsoft Excel format or as Web Forms. Some may be filled out online and then printed or saved and emailed while others may need to be printed first.

Web Forms that exist on individual departments' web sites usually should be submitted electronically.

Contact uvaforms@virginia.edu to request a form be updated or added to or deleted from the directory.

Instructions

  • Enter a word or phrase to search for.
  • Use a comma to separate multiple words/phrases.
  • Choose a department.
  • PDF forms need to be viewed or filled out on-line using Adobe Acrobat Reader version 3.0 or later and then printed.
  • PDF forms with data cannot be saved or e-mailed.
